1. Between Now and Dec 31, enter a membership fee of $40 in the amount field.

2. In the notes field, enter the Full Name and Email of the person to be enrolled.

Enrollment is for Gold Membership effective January 1, 2025 through December 31, 2025. Please see the HCPG Constitution & Bylaws for full membership details.
